During radiotherapy, chemotherapy, abdominal infection, or any major surgery, the dead cells in the body start releasing a chemical called serotonin directly into the blood. Ondansetron is a potent and highly selective 5-HT3 receptor antagonist. Its precise mode of action in controlling nausea and vomiting is not fully known. Chemotherapeutic agents and radiotherapy can release 5-HT in the small intestine, initiating a vomiting reflex via 5-HT3 receptors on vagal afferents. Ondansetron blocks this reflex. The release of 5-HT in the area postrema (brainstem) may also contribute to emesis through a central mechanism. Thus, ondansetron likely manages nausea and vomiting by antagonizing 5-HT3 receptors in both the peripheral and central nervous systems. The mechanism for post-operative nausea and vomiting may involve similar pathways.

Onaseron Oral Solution may cause a serious reaction when used with certain medicines like pain killers (acetaminophen, tramadol, ibuprofen), anti-anxiety (pregabalin, lorazepam), and antiparkinson medicine (apomorphine).
The use of Onaseron Oral Solution is restricted in people with hypokalemia (low potassium) and severe liver disease.
